"INCIDENT_REPORT": "Incident Description: A homeowners' association management company hired a cleaning contractor to clean a Toho-owned mainline without first notifying Toho. Horizon, the cleaning company, used a vacuum truck to jet the mainline. During the process, the hose became lodged in the manhole, causing a discharge of approximately 2,500 gallons of wastewater. Toho assisted with the response by setting up a bypass and using tankers and vacuum trucks to draw down the system so the lodged hose could be removed. The mainline was then lightly cleaned and televised by CCTV. The spill reached a nearby storm drain. Approximately 15,000 gallons were recovered from the retention pond.
