  "INCIDENT_NAME": "B02MH031 Manhole Overflow",
  "INCIDENT_REPORT": "On August 31, 2026, at approximately 10:10 a.m., the Collections System Manager was notified by City Administration of a wastewater overflow occurring on 19th St NE, beginning at City of Bradenton manhole B02MH031 and extending to Riverside Dr E in Bradenton, Florida. Lift Station staff responded immediately to investigate. The overflow was caused by a pump failure at the lift station. The station's radio telemetry system did not transmit an alarm notification, and the audible alarm was found to be inactive. Staff reset the pumps and pumped down the lift station, stopping the overflow. The estimated discharge was 1,500 gallons and impacted the roadway and storm drain system, with the wastewater entering the Manatee River via the storm drain system. Cleanup actions included spraying down the impacted roadway, dispatching a vacuum truck to attempt recovery of the wastewater, and liming the impacted area. The audible alarm switch was replaced with a momentary switch to restore proper alarm functionality. The termination strip within the radio telemetry system was replaced to re-establish reliable communication and alarm notification. The lift station pumps were serviced to address the pump failure and ensure proper operation. The audible alarm and radio telemetry system were tested following completion of the repairs to verify proper operation and communication.",
